For these informational pieces, I like to address common questions from clients. Today we are going to discuss the forms required to sign up for Medicare Part B after the age of 65. Most Medicare recipients are delaying their Medicare options and remaining on employer health plans until they reach the Social Security full retirement age.
When Medicare Part A recipients decide to retire, they will be required to fill out two forms: the Application For Enrollment In Medicare Part B and the Request for Employment Information form. These two forms must be sent together to the local Social Security office. It is important to note that the Medicare Part B effective date should be written in the remarks section of the application and the Request For Employment Information form must be completed by your Human Resources department.
